Responsibilities:
- Deliver compassionate and holistic care to terminally ill patients in hospice settings
- Assist with activities of daily living and provide personal care services
- Support patients with mobility, transfers, and positioning
- Collaborate with healthcare professionals to implement individualized care plans
- Monitor patient conditions and report any changes to the healthcare team
- Provide emotional support and companionship to patients and their families
- Maintain a clean and safe environment for patients
- Document patient care activities accurately and timely
Skills:
- Proficiency in home community care and assisted living environments
- Ability to lift and assist patients with mobility needs
- Experience in dementia care and memory care support
- Knowledge of catheter care procedures
- Familiarity with meal preparation for specialized diets
- Strong organizational skills to manage systems effectively
- Commitment to providing compassionate care in end-of-life situations
