Our company is seeking a Wellness Coordinator to oversee the care provided by our caregivers to the residents of Concord Place Retirement Community in Northlake.
Position Summary
The Wellness Coordinator will perform duties that ensure that the oversight of the caregivers contributes to the client's comfort and safety in the home. The Wellness Coordinator will ensure that the standards of ALC Home Services are upheld and that all organizational directives and IDPH regulations, within the purview of this position, are met.
WELLNESS COORDINATOR RESPONSIBILITIES
- Scheduling of staff to meet client needs within their area of responsibility as designated.
- Scheduling of all caregiver assignments within the area of responsibility as designated.
- Meeting with clients and families for care plan development.
- Supervision of all caregivers including, but not limited to, ensuring all services are delivered and the services are documented.
- Discipline/re-education of staff as necessary for not meeting the standards/policies set forth by ALC Home Services.
- Ensures the delivery of care meets the standards of excellence required by ALC Home Services.
- If necessary, must be able to cover assignments if no caregiver is available to ensure no services are missed.
- Implementation of all policies set forth by ALC Home Services in their area of responsibility.
- Creates and sends monthly invoices to client/families and receives all payments or ensures that payment has been received by ALC Home Services.
- Follow up with client/families for non-payment of services and enforce terms of signed contracts with ALC Home Services within their designated area of responsibility.
- Responsibility for submitting time sheet information so that employees are paid correctly and in a timely manner.
- Ensure that caregivers are aware of and following the policies set forth by ALC Home Services.
- Conflict resolution with clients/families or staff.
- Maintains appropriate communication with supervisory staff and with all members of the interdisciplinary team.
- Treats clients and families with respect.
- Maintains confidentiality per HIPAA and ALC policies.
- Exterior marketing to ensure continuing presence of ALC Home Services to physicians, hospitals, etc.
- Accepts and fulfills assignments with the agency and exercises judgment in accepting assignments.
- Attends required in-services, meetings, and training programs per agency policy.
WELLNESS COORDINATOR QUALIFICATIONS
- Demonstrate ability to read, write, and follow/interpret IDPH regulations.
- Demonstrate the ability to develop a written Plan of Care.
- Be self-directed and work with minimal supervision.
- Demonstrate dependability, tact, patience, and the ability to follow instructions.
- Demonstrate good interpersonal and communication skills.
- Have experience in the coordination of home services or related experience/education.
- Previous supervisory experience or related experience/education preferred.
Job Type: Full-time
Pay: $25.00 - $27.00 per hour
