Intake/ Home Care Coordinator - Weekend Package

32 hours/weekend (Friday-Monday)
Friday 8AM-5PM
Saturday 8AM-4PM
Sunday 8AM-4PM
Monday 8AM-5PM
This person will be coordinating in-home care for internal and external patients/referrals. Working to schedule initial care appointments at home, the set up and delivery of equipment and authroizing insurance for home care. Responsible for coordinating services for patient/customer with UnityPoint at Home; utilizing other members of the home care coordination team as necessary to coordinate services outside of the specialized service line.
This role can work from home after 12 week orientation/training period- WEP training period is Mon-Fri 1st shift hours
- Must sit in Des Moines or surrounding area
Responsibilities:
This person will be coordinating in-home care for internal and external patients/referrals. Working to schedule initial care appointments at home, the set up and delivery of equipment and authroizing insurance for home care. Responsible for coordinating services for patient/customer with UnityPoint at Home; utilizing other members of the home care coordination team as necessary to coordinate services outside of the specialized service line.

Processes all incoming work orders, interviewing client/referral source to secure demographic and financial and qualifying data, and securing verification and authorization of benefits as appropriate.

Specializes in one or more of the agency’s service lines, yet is responsible for having a basic knowledge of all service lines.

Directs inquiries and referrals to the appropriate individual and/or service line. Coordinates incoming calls from both referral sources and patient/customers.
Qualifications:
- One year of medical experience; preferably in community home health or home medical equipment.

High-school degree - Associate degree preferred

Completion of Coding courses are highly desirable.
- Has previous health care or home care related Intake/Coding and Customer Service experience.

Above average data entry and communication skills.
- Knowledge of insurance requirements for pharmacy dispensing to include NDC codes and AWP rates.
