Position Summary:
Are you a detail-oriented RN with a passion for making a difference in the lives of residents? Join our exceptional team at The Osborn as a Resident Assessment Coordinator! In this dynamic role, you will support the Director of Resident Assessment in managing the daily operations of the Pavilion. Your responsibilities will include completing MDS assessments and coordinating the MDS process with our dedicated Interdisciplinary Team. When the Director is unavailable, you will step in to ensure seamless continuity of care.
As a crucial member of our organization, you will work closely with the Director of Resident Assessment, Director of Nursing, Pavilion Administrator, and other departments to drive the long-term, strategic growth of The Osborn and its mission of excellence in resident care.
Key Responsibilities:
- Complete MDS assessments and coordinate the MDS process with the Pavilion Interdisciplinary Team.
- Assist the Director of Resident Assessment with daily operations and assume their responsibilities when needed.
- Collaborate with various departments to ensure the highest standards of care.
- Lead and participate in team meetings, fostering a collaborative and positive environment.
- Ensure compliance with Medicare Skilled Care Guidelines, MDS 3.0 regulations, and Quality Indicators and Measures data.
Knowledge & Skills:
- Extensive knowledge of Medicare Skilled Care Guidelines and MDS 3.0 guidelines and regulations.
- Familiarity with Quality Indicators and Quality Measures data.
- Strong communication skills to effectively collaborate with all departments and team members.
- Leadership abilities to guide and inspire the team.
- In-depth understanding of Gerontological Nursing.
- Positive attitude and role model qualities for nursing staff.
- Proficiency in computer literacy and training on new patient care systems.
- Ability to make independent decisions and adapt to new methods and principles.
Qualifications:
- Current NYS Licensure to practice as a Registered Nurse (RN).
- Ability to work effectively with others in the organization.
- Patience, tact, enthusiasm, and a cheerful disposition.
- Knowledge of basic nursing care techniques.
- Strong leadership skills and the ability to relate information concerning a resident’s condition.
