Role Description: Under indirect supervision and working interdependently with physicians, nurses, behavioral health providers, community health workers and other healthcare providers, provide a broad range of nurse practitioner services following established policies and protocols.
DUTIES AND RESPONSIBILITIES:
- Provide comprehensive clinical services to an assigned panel of patients including but not limited to collaborative care planning and management, ambulatory care, chronic disease management, taking health histories, performing assessments, implementing nursing interventions, and conducting health education and counseling via phone, video, or chat using our software platform
- Perform scheduled and unscheduled episodic urgent care and general medical care visits for patients’ minor, acute illnesses via phone, video or chat
- Perform assessment/data collection in an ongoing and systematic manner, focusing on social determinants of health and physiologic, psychological, and cognitive status
- Develop and manage individualized, patient-centric treatment plans based on patient assessments in accordance with established nurse practitioner protocols for all assigned patients.
- Collaborate and partner with clinical and non-clinical care team members to manage a patient’s health needs holistically; champion the integration of behavioral health and social-emotional wellness into the model of care for each patient; attend weekly interdisciplinary care team conferences to ensure appropriate care plan management and follow-up of patient panel
- Provide teaching and emotional support to patients and families
- Responsible for ensuring timely and appropriate medical care in order to avoid hospitalization and use of the emergency department services
- Work with the patient’s primary care provider, specialists, hospitals, diagnostic services, pharmacy services, clinics, and community-based organizations
- Identify patients and refer members to the care team support staff for coordination and follow-up with members to ensure support services and supplies as required that can improve the health status of the patients are obtained and managed
- Perform post-discharge and transitions of care visits for members of your patient panel within prescribed times following discharge from a facility
- Comply with all time-based service level policies (e.g. time to complete assessments for newly assigned patients, time to review and respond to communications and incoming requests related to patient care, time to complete documentation)
- Utilize written guidelines/protocol for clinical decision making and management
- Provide medication reconciliation services and direct reminders to patients via phone calls, messages, and virtual visits to increase compliance with their treatment plan, medication adherence, and address any gaps in care
- Recommend, participate in and work to ensure the success of our efforts in improving cost effectiveness and quality of care provided to patients
- Apply assessments, planning, implementation, and evaluation in formulating diagnoses, treatment plans and interventions
- Prepare timely, legible, and complete documentation of all patient care services as required by law, regulation, and established policy
- Demonstrate an awareness of and sensitivity to patient/family rights
- Participate in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices
- Ensure compliance with legal issues including but not limited to patient confidentiality and risk management; ensures compliance with NCQA, federal, state, and local regulations
- Exhibit a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals; build an environment of trust and compassion for all patients
- Adjust to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality
- Perform other related duties as assigned by management
QUALIFICATIONS:
- Graduate from an accredited school of nursing
- Board-certified Nurse Practitioner or Physician’s Assistant with an unrestricted license to practice in the State of Illinois
- For Nurse Practitioners, an AANC or AANP certification
- 2-4 years experience providing medical services to patients with chronic conditions (e.g. diabetes, hypertension, kidney disease, COPD); experience in an interdisciplinary primary care team setting with a focus on behavioral health condition management a plus
- Specialized education and training in one of the following areas: Behavioral/Psychiatric Health, Pediatric and/or Adolescent care, Geriatric Care, General Family Practice, Women's Health
- Demonstrated interest in combating systemic racism in health care and serving vulnerable communities with complex care needs
- Excited about how technology can support your work and help drive the ongoing evaluation toward new and better care
- Experience with care coordination, remote patient monitoring, telemedicine and/or virtual primary care programs desired
- Ability to understand and adhere to established policies, procedures, and protocols
- Active Illinois, DEA and Controlled Substance license/registration required or application in process
- US work authorization
- Able to work collaboratively with multiple health professionals in a busy and complex environment using tact, diplomacy, and discipline
- Ability to manage priorities and workflow
- Strong interpersonal skills; ability to effectively communicate with people at all levels and from various backgrounds; demonstrated compassionate and caring nature
- Multilingual skills are a plus
- Availability to work in an office environment if required
- Wiling to cover some on call nights and/or weekend days as needed
