Job Summary:
The purpose of the Remote Care Coach is to engage chronically ill patients through monthly phone calls, around best practices for self-management. The Remote Care Coach will be required to engage each patient in their assigned practices for at least 20 minutes each month. You will utilize electronic medical records and information provided by electronic devices (ie. blood pressure cuffs, weight scales, etc.) deployed to the patient's home. The data retrieved from our electronic devices and the notes from our team is shared with physicians in our client practices to improve continuity of care.

- Must be available at least 35 hours per week
- This is a work-from-home position. You must have a computer with at least 8 gigs of RAM, and reliable access to the internet to use our software. Must have a headset, or obtain one. No equipment is provided.
- Staring pay = $16 to 18 for MA's and $18 to $23 for LPN's
- Positions are non-benefited (no health insurance)

Essential Responsibilities:
Collaborates with the Nurse Care Manager to discuss assignments and strategies for patient engagement
Meets realistic productivity goals
Documents details of patient conversations and findings
Escalate real-time or emerging patient health events appropriately
Encourages patients to frequently utilize the electronic devices they have been given to monitor their conditions
Maintains timely, ongoing verbal communication with patients to provide support and build lasting relationships
Utilizes the Healthmote platform to document length and details of each patient conversation
Treats each patient in a manner that preserves and supports patient confidentiality, patient rights, and patient dignity
Learns and utilizes persuasive coaching techniques to motivate patients to improve self-management of their conditions.
Performs other duties as required
Qualifications:
Completion of an approved Medical Assistant training course
Basic knowledge of medical terminology
Able to understand vital signs accurately
Ability to use personal computer for documentation and retrieval of pertinent medical history
Proficient in reading and writing skills in English
Excellent communication and customer service skills
Ability to work independently
Experience Required:
Minimum of 2 years of LPN or MA experience.
High school diploma or GED (preferred but not required)
Experience in a medical environment required
Preferred: Chronic Care Management, transitional care management, or CPC+ experience
