Seneca Nursing and Rehabilitation Center is seeking an experienced Full Time RN MDS Coordinator to join our team! We are located in Waterloo; NY and we are part of Hurlbut Care Communities that has 11 Nursing Homes across 4 counties.

The primary purpose of this position is to provide Patient Centered Care, be the patients advocate and to coordinate the RAI process assuring the timeliness, and completeness of the MDS, CAA’s, and Interdisciplinary Care Plan. The ideal candidates must be devoted and compassionate about their chosen career and is passionate about patient care. The RN MDS Coordinator is the patient advocate and bridges the communication between patient, family, and facility staff.
Responsibilities of MDS Coordinator:
*Responsible for case mix with emphasis on meeting or exceeding budget expectations.
*Assesses and evaluates residents for Medicare Part A coverage prior to and upon admission
*Schedules screening and determination of coverage for all residents who may qualify for Medicare Part A
*Completes and maintains Minimum Data Set (MDS) compliance per CMS guidelines.
*Coordinates scheduling for annual, quarterly and significant change MDS and timely submission to government agencies.
·*Performs medical record review as requested by CMS/HMO’s using their guidelines.
*Oversees/assists nursing staff responsible for maintaining compliance of MDS.
*Trains and updates staff in MDS and care planning.
*Develops and updates assigned policies and procedures; Ensures nursing department compliance with Medicare, Medicaid, and other insurance reimbursement regulations, including but not limited to documentation
*Monitors and guides the completion of assessments in a timely manner
* Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of validation
*Schedules and facilitates care plan conferences Facilitates monthly QM meetings
*Update and revise patient care plans as needed
Qualifications of MDS Coordinator:
- Must hold a Nursing Degree from an accredited college or university
- A current active RN license in NYS
- At least 1 year’ experience in Long Term Care as an MDS Coordinator.
Skills and Qualities of MDS Coordinator:
- Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
- Excellent written and verbal communication skills and critical thinking abilities,
- Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
- Ability to organize and prioritize.
