Responsibilities
- As a care team member you will perform routine primary care nursing tasks, including: Vital signs ,Point of care tests (glucometer, etc)
- Maintain patient medical records
- Documentation consistent with state regulation
- As a care team member, you love utilizing motivational communication techniques and strategies, to help drive positive health behavior change and improved patient outcomes
- The MA/RN/LPN plays a vital role in educating patients on how to manage their acute or chronic medical conditions, with a goal to maintain health, wellness, and to avoid emergency room utilization and hospitalization.
- Significantly impact longitudinal patient engagement in RPM program(s)
- Provide preventive health and disease management education and coaching.
- Collaborate with care team to help ensure positive patient outcomes
- Ability to provide instruction to patients on use of the portal
- Determines and confirms eligibility of patients who are able to receive the CCM services
- Provides basic health information and education to patients through the portal and telephonic sessions
- Completes 200-20 minute claims monthly, appropriately document the time logs and CCM documentation
- Participates in all trainings and meetings associated with the CCM program
- Knowledge of eCW basics for insurance, appointment setting, referral management and some clinical documentation
Qualifications:
- Consistent professional conduct and meticulous attention to detail
- Excellent verbal and written communication skills as well as excellent interpersonal skills with patients, staff, and other health care professionals
- Bachelor of Science in Nursing (BSN) or higher educational attainment from an accredited program
- 1 year experience in case management, preferably within a healthcare or hospital setting
- Minimum 1 year of experience as a Chronic Care Manager

Pay: $25.00 - $35.00 per hour
