We are seeking a compassionate and experienced Registered Nurse (RN) to join our team as an Assisted Living Program (ALP) Nurse. This role presents a unique opportunity for a dedicated nurse with home care experience to transition into an administrative position within our organization. As an ALP Nurse, you will play a key role in overseeing the clinical operations of our assisted living facility, ensuring the highest standards of care are maintained for our residents.
Responsibilities:
1) Maintain and promote working relationships with CHHA and DOH.
2) Perform semi-annual and annual UAS-NY Assessments for ALP residents.
3) Review residents’ status weekly to assure Director of Personal Care has addressed/solved any or all issues.
4) Update Assisted Living residents' care plans as care requirements change, but in no event less frequently then once every six months.
5) Consult with case management and agency administrator concerning new admissions and discharges of residents.
6) Completes UAS-NY assessment for all new ALP admissions to determine level of functioning upon admission.
7) Oversee the notification of physicians with changes in physical/mental conditions of all residents.
8) Document records to include resident assessments, communications and all ALP
information.
9) Promotes positive working relationships with other departments.
10) Attends meetings and continuing education sessions including those necessary
for maintaining licenses and certification.
11) Conduct PCA and HHA training programs, including methodologies Nurse Aide Transition, Competency Evaluations, PCA Alternative Demonstration.
12) Conduct quarterly Quality Assurance meeting/audits and reports on quarterly
Quality Improvement.
13) Assist when needed to destroy narcotics on a quarterly basis.
14) Assist in the response to DOH investigative reports of complaints and/or
violations concerning all residents.
15) Perform such tasks as may be deemed necessary by management in order to
promote the health and safety of the residents.
16) Tracking of Hep B and administration of vaccine to employees. Turn completed paperwork into Human Resource Department for employee medical file.
Qualifications:
Current RN license in NY
1-2 years experience in home care or assisted living
Job Type: Full-time
Pay: $32.00 - $38.00 per hour
