Overview:
The Medication Specialist will provide partnership, administrative support and accountability to Outreach programs in the office and virtually. They will assist Outreach teams in capturing all medication change opportunities and support the follow through of the medication change process. They will possess knowledge of medications, safe storage and health insurance systems to help the Community Support Teams (CST) and Recovery Services (RS) teams mitigate risks by adhering to best practice and company policy. They will help manage systems of communication and documentation between the clinical staff, providers, and the pharmacy, providing partnership to each by tracking changes to ensure compliance while maintaining a professional and efficient relationship with internal and external stakeholders.
Responsibilities:
- Provide specialized medication administrative and clerical support for CST and RS levels of care; i.e. tracking appointments; coordinating between pharmacy, clinic and teams; following up on medication changes; auditing physical Medication Administration Record (MAR) and med boxes to ensure compliance; documentation reminders, partnering with TLs and teams around medication issues, assisting with audits with Trilogy and clinic Electronic Health Record (EHR); completing critical incident reports as needed
- Provide support to Team Leaders and Program Managers with team operations around medications, safe medication storage, distribution and reduce opportunities for error; i.e. regular attendance at team meetings, maintenance of medication lists, completion of tracking forms and logs, etc.
- Act as a liaison between the clinic, pharmacy and teams to ensure medications are being followed up on in a timely manner and clients are getting the correct medications.

Ability to quickly grasp systems and build relationships with multiple types and levels of staff;

Ability to understand the medication policy at Trilogy and support program outcomes by developing workflows to complete tracking in a timely and accurate manner;

Ability to navigate and enter data in various EHR systems, Microsoft Suite, etc.;

Assist with medication reconciliation, compliance of medication sheets, and tracking changes, injections that still need to occur.
- Assist with the controlled substance process, which includes sign-in/sign-out of medications per policy.

Assist in retrieving and organizing updates and needed follow-up in daily team meetings.

Attend weekly risk meetings about medication errors observed throughout the agency; provide administrative and collaborative support to the Medication Error Reduction Taskforce.

Actively participate in daily group chats with assigned teams to support in navigating questions regarding medication changes, medications, etc..
- Support teams in tracking client medical appointments and ensuring that clients are following up with providers i.e. psychiatry, primary care, and specialists.

Represent Trilogy in a positive and professional manner; and communicate effectively and appropriately to others about potential organizational issues.

Support with communication to funding bodies, Strategy team, RNs, etc related to client outcomes and goals achieved with greater medication independence.

Perform other related duties and/or projects and participate in committees/workgroups as assigned.
Qualifications:
- Thorough knowledge of psychiatric and common medical medication terms, contradictions, side effects, etc through a demonstrated work experience as a pharmacy technician or LPN with experience monitoring medications (2+ years)

Thorough knowledge of Microsoft applications (Outlook, Teams, Word, Excel, etc.), report generation, electronic health records, and other database management tools

Demonstrated work experience with medication compliance, processes, policy, safe storage and risk reduction

Ability and experience in navigating EHRs
