Eliot is currently seeking a Program Manager for the new Lowell Forensic Program for Assertive Community Treatment (PACT) Team.. The ideal candidate will have an understanding of the unique needs and history/background of the population and knowledge of placement options. A proven experience in community-based work with adults with severe and persistent mental illness are a must.
PACT is an evidenced based multidisciplinary outreach mental program that serves adults with serious and persistent mental illnesses in the community. PACT clinicians work as a team to collaborate with consumers to develop treatment interventions that assist them in their recovery process.
Responsibilities:
- Manage and oversee the day to day operation and overall service delivery of program in accordance with all applicable laws, regulations, agency and program policies and procedures.
- Provide clinical and administrative supervision to all staff in the program.
- Coordinate and oversee all case management related services for persons served in the program. This include assisting persons served with managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.
- Create a culture of hope, recovery and resiliency at the program to promote persons strengths, preferences and independence. Ensure that each environment is recovery focused and encourages every person to achieve goals and transition to more independent living. Ensure that each program supports persons served in accessing and utilizing natural supports and community resources.
- Provide support and assistance with management of medical symptoms and conditions in accordance with person’s needs.
- Coordinate supportive services as needed such as hospice and spiritual care for persons and their families.
- Ensure that the program provides and/or promotes access to meaningful day-time activities that meet the persons’ treatment plan goals and individual needs.
- Participate in the Assessment process and the development of Treatment Plans.
Qualifications:
- Current Massachusetts clinical license including LICSW, LCSW, LMHC, LRC, LADCI, RN or OT, preferred.
- Candidate must possess at least 2 years of experience working with behavioral health and/or justice involved individuals. Ability to consistently meet all necessary background and credentialing requirements.
- Must have a strong commitment to mental health recovery. Must have ability to provide supervision and leadership to direct care staff.
- Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses and respect for their preferences for treatment.
- Must have ability to provide flexible services that are community based and focused on team work.
- Must have an understanding of the unique needs and history/background of the population being served which includes persons who have limited social skills and experiences with histories of unsuccessful community tenure that can be due to minimal community exposure as a result of long term hospitalizations or incarcerations,
Schedule: Monday - Friday, 9am-5pm
Location: Tewksbury, MA
Salary: Based on license and experience
6% bilingual salary increase if fluent in the needed language capacity.
The sign-on bonus is prorated for positions under 40 hours per week
