Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. POSITION SUMMARY
The Registered Nurse (RN) Charge Nurse provides skilled nursing care in the inpatient hospice facility to hospice patients in accordance with the individualized plan of care developed by the patient and/or family, physician and hospice team.
This position will be based in the Hospice inpatient unit in the Heritage Valley/Beaver Medical Center. This is a full time position working 3-12 hours shifts with a weekend and holiday rotation.
QUALIFICATIONS
- Current RN license in the state of Pennsylvania
- Minimum of two [2] years experience in patient care preferred
- Demonstrates excellent clinical skills
- Solid computer skills, including Word
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
RESPONSIBILITIES
1. Works with the Interdisciplinary Group (IDG) to develop, coordinate, and update the individualized plan of care for hospice patients/families cooperatively with the patient and/or family, physician and hospice team
2. Conducts a physical examination and nursing assessment of the patient on each shift and as necessary
3. Serves as the charge nurse for designated patients by coordinating patient care, evaluating the effectiveness and appropriateness of the plan of care, and making changes when necessary
4. Provides personal care such as (but not limited to): bathing, grooming, dressing, shaving, feeding; assessing vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation
5. Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death
6. Completes all computer and/or paper documentation requirements within the timeframes specified
7. Works shifts as needed, including weekend and holiday rotations.
