What You'll Do:
- Conduct supervisory Client assessments.
- Develop and implement individualized care plans.
- Collaborate with other Health Care Providers
- Ensure compliance with healthcare regulations.
- Facilitate effective communication among Management Team, patients, families, and healthcare providers.
- Foster a positive and supportive environment for clients, families, and team members.

Qualifications:
- Active LPN license in Georgia
- Minimum of 3 years of home care experience, with at least 1 year in a supervisory role
- Strong clinical skills and knowledge of home care practices
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic attitude toward clients and their families.
- Reliable transportation and valid driver's license.
