The Home Health - RN Case Manager will be responsible for providing skilled nursing care to patients in their homes. The RN Case Manager will work closely with the patient's physician to develop and implement a plan of care that meets the patient's needs. The RN Case Manager will also be responsible for coordinating care with other healthcare providers and ensuring that the patient receives the appropriate services.
- Assess patient's condition and develop a plan of care
- Provide skilled nursing care to patients in their homes
- Coordinate care with other healthcare providers
- Ensure that the patient receives the appropriate services
- Educate patients and their families about their condition and the plan of care
- Document patient care and progress
- Communicate with the patient's physician about changes in the patient's condition
- Participate in case conferences and team meetings
- Current RN license in the state of Texas
- Minimum of 1 year of experience in home health
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Valid driver's license and reliable transportation
