The Care Manager is a member of the care team that facilitates and provides high quality and holistic healthcare to high-risk patients with an emphasis on patients with behavioral health and/or substance use disorder diagnosis through a client-centered approach. The RNCM works closely with the patient’s primary care provider, substance use disorder professionals, and behavioral health providers. Qualified candidates will have a current RN license. Effective communication, organization, efficient time management skills, and the ability to successfully multi-task are essential to your success in this position. Collaborates with all MyCare team members to provide positive service experiences for patients.
SPECIFIC DUTIES AND JOB FUNCTIONS:
- Examines, assesses, and reviews patient symptoms for signs and symptoms of substance dependency and use.
- Collaborates with primary providers and treatment teams to develop a treatment plan and communicates and executes treatment plan with the patient, family, and primary care team.
- Prepares/assists with care team “huddle” and pre-visit planning utilizing data analytics.
- Participates in meetings and committees as coordinated by MAT Team Lead, including MAT staff meetings.
- Makes themselves available to patients at all MyCare locations and is present during patient encounters as needed.
- Supports and empowers patients throughout health care continuum to include holistic counseling, problem solving “barriers to care” issues and coaching clients as needed re: treatment and medication adherence, healthy life choices, and self-help behaviors.
- Responsible for facilitating appropriate care and follow up to high-risk and transition of care patients.
- Utilize data analytics to monitor patient outcomes and take appropriate follow-up action with patients and care team.
- Performs nursing assessment/medical triage of acute medical issues and medication side effects and refers or monitors appropriately.
- Provides chronic disease and preventive screening education to clients/families/care givers/partners, internal staff and external agencies in an organized and appropriate manner.
- Works in collaboration with care team to assist clients in accessing support services by serving as a liaison with other community agencies.
- Works in collaboration with referral specialist to manage patient referrals and follow-up to other specialty medical, mental health, substance use disorder and community services.
- Prepares/assists with monthly Quality Improvement program activities and facilitates development and ongoing activities for consumer involvement.
- Participates in meetings and committees within MyCare and in the community as requested.
- Maintains electronic health record by updating patient information as appropriate.
- Participates in local and national training and education to foster continued professional growth and assure high quality care delivery.
- Provides assistance with direct nursing interventions such as venipuncture, injections, vaccinations and other identified nursing responsibilities and performs additional duties as assigned, including but not limited to assisting the Primary Care Provider with annual Health Risk Assessments.
- Demonstrates HIPAA compliance and utilizes discretion dealing with confidential or patient care matters.

MINIMUM QUALIFICATIONS:
- Registered Nurse with license recognized by the State of Michigan Board of Nursing. Bachelor of Science in Nursing preferred.
- 1 year experience working with Medication Assisted Treatment or Substance Use Disorder Treatment
- 3 years experience in healthcare setting required, Federally Qualified Health Center (FQHC) preferred.
- Proficient with Microsoft Office Suite including excel, outlook and word.
- Knowledge of electronic health records, AllScripts/Veradigm preferred.
