The Lead Care Manager (LCM) operates as part of the member’s multi-disciplinary care team and is responsible for coordinating all aspects of ECM. Under the direction of the ECM Director and/or Clinical Consultant, the LCM will assist in the development of treatment plan(s) that are appropriate to the needs and desires of the consumer. The treatment plan(s) will aim to promote recovery and stability for community living and helping the client to achieve higher levels of function. This position is the primary care manager for the ECM member and will be responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services.

Duties Include:
- Oversee provision of ECM services including outreach and engagement.
- Provide person-centered intensive case-management services and developing care plan.
- Offer services where the member lives, seeks care, or finds most easily accessible and within ECM guidelines.
- Connect member to other social services and provides supports, including transportation.
- Advocate on behalf of members with health care professionals.
- Use motivational interviewing, trauma-informed care, and harm-reduction approaches.
- Coordinate with hospital staff on discharge plan.
- Accompany member to office visits, as needed and according to ECM guidelines.
- Monitor treatment adherence (including medication).
- Provide health promotion and self-management training for assigned clients
- Coordinate client care with other providers both within and outside the agency.
- Attend and participate in all in-service training as necessary.
- Responsible for timely completion of documentation in the client’s clinical record
- Responsible for meeting assigned minimum productivity requirements on a monthly basis.
- Comply with all Company policies and procedures as applicable.

Requirements:
Minimum Qualifications:
- Bachelor’s degree in social work, Gerontology, Health Administration, Human Services, or related field required.
- Minimum two years of experience working with older adults.
- Fluency in English (bilingual in Spanish strongly preferred).
- Previous experience working with an ethnically and economically diverse population.
- Previous care management experience is strongly preferred.
- Patient Coordination and Healthcare experience are strongly preferred.
Preferred Qualifications:
- 3-5 years of experience providing home health and/or social services case management services to low-income populations with one or more of the following: complex chronic conditions, high utilizer of emergency room and tertiary health care services, severe mental illness, and/or homelessness preferred. Bachelor’s Degree from accredited college/university in a related field (i.e., social work, psychology, or sociology).
Pay Range: $23.00 - $30.00/Hour (Determined by Experience & Qualifications)
