North Texas Behavioral Health Authority (NTBHA), based in Dallas, is responsible for developing a collaborative system of care for children, adolescents and adults with severe mental illness and other serious emotional disturbances, as well as those experiencing a substance use disorder.
We are responsible for developing a collaborative system of care for individuals in crisis in the NTBHA service area. We are focused on trauma-informed, resilience-oriented care in all services provided.
This position’s actions and duties must align with NTBHA values and goals, regulations, policies and procedures, and meet all documentation standards and regulatory requirements.
NTBHA is seeking a person with excellent organizational and clinical skills to provide continuity of care and coordination.
Hospital Based Early AM or Late PM Care Coordinators (CCs) will bridge the gap in services where barriers once were between inpatient and outpatient providers and assist by providing access to care for the individuals we serve.
The role of a Hospital Based Early AM or Late PM CCs is to perform complex coordination and assessments to assist in identifying strength and needs to eliminate barriers to connect individuals with mental illness and/or a co-occurring disorder transitioning from crisis services to outpatient services.
The Hospital Based Early AM or Late PM CCs actively participate in treatment team meetings to determine best treatment options for the individual and assist in developing discharge plans. Discharge and diversion plans include coordination, referral and linkage to community-based services which includes but is not limited to: transitional Crisis services (LOC5), Assertive Community Treatment (ACT), Rapid Assessment & Prevention Programs (RAP), Early Onset (EO) Psychosis program and outpatient mental health services, or substance use disorder services such as OSAR and Peer Support Services.
Our Hospital Based Early AM or Late PM CCs will provide aftercare follow up calls after discharge to the individuals and providers to assist with increasing the outpatient connection activity and engagement in services. The CCs work with the highest utilizers to ensure that the individual is supported for outpatient services when discharging from an acute setting or crisis situations to the community. CC’s will develop and maintain rapport using Motivational Interviewing and various curriculums such as Illness Management and Recovery (IMR) in working with individuals.
CC’s will collaborate with internal departments, law enforcement, state and local hospitals, community agencies and other organizations.
This position requires flexible work hours to meet needs of the duties assigned. This position will require shifts within early morning and/or late evening hours. This role is responsible for adherence to and compliance with NTBHA policies and procedures and state regulatory codes.
Education required:
Minimum of a bachelor’s degree from an accredited four-year college or university in a social, behavioral or human service field.

A master’s degree is preferred.

Must meet TAC criteria for QMHP (Qualified Mental Health Professional) status.

Typical majors include sociology, social work, psychology, counseling or other TAC-approved majors.
Experience required:

At least 1-2 years experience with direct care of persons with serious mental illness and/or substance use disorders. Preference given to experience in psychiatric crisis service settings.

Experience participating as a member of a treatment team that develops and monitors treatment plans for persons with chronic & serious mental illness.

Current knowledge of local community resources, providers and social supports for homeless and indigent populations within the NTBHA service area.

Understanding of and experience with HHSC programming and systems preferred, including Texas Resilience and Recovery, CMBHS, and CARE.

Possess and maintains a valid driver’s license. Pass and maintain a clear background record as required for healthcare organization under state and federal contracts.

Work with other NTBHA employees, members of partner agencies and organizations, as well as the target population and their families in a constructive and efficient fashion.

Ability to work/communicate with individuals with a serious mental illness and/or co-occurring disorder.

Prepare, create, and maintain records, files, documents, reports, and correspondence both within NTBHA and partner agencies and organizations.

Be respectful of diverse views and approaches, and maintain professionalism and acceptance toward all employees, patients and visitors.

Ability to be flexible with work hours to meet the needs of the facility and/or duties assigned.

Provide timely responses to calls and/or emails from various professionals within NTBHA and outside NTBHA.

Bilingual ability preferred but not required.

Advanced Microsoft Office Suite skills - Outlook, Word, Excel, PowerPoint, etc. with an ability to become familiar with company-specific programs and software.

Must be able to work on and have knowledge of a PC (personal computer).
