**Seeking a Weekend On-Call Registered Nurse from 4:00 PM Fridays until 8 AM Mondays**
We are seeking a skilled Registered Nurse (RN) to join our amazing and expanding team! As an on-call RN, you will provide care and support to patients during their final stages of life. You will also help support their families and collaborate with healthcare professionals.
This position will be responsible to cover the following geographic areas: Allegheny County, Washington County, Westmoreland County, Fayette County, Greene County
In this role, the ideal candidate will have profound knowledge and skills in caring for the terminally ill. We seek a compassionate nurse with excellent interpersonal skills who possesses a thorough understanding of hospice philosophy and care. The ability to educate families, provide care aligned with patient’s values, and provide support for patients and families are vital in this position. Our purpose is to improve our patients' quality of life during their final days with dignity, comfort, and support.
Responsibilities + Duties of Hospice On-Call Nurse
- Responding to calls from answering service and voice mail system in a timely manner
- Triaging patient related calls by determining the needs of the caller and then responding in an appropriate manner either by phone or with a visit
- Implementing changes to the plan of care as needed with interdisciplinary collaboration with the support of the hospice medical director and / or the attending physician
- Documenting and communicating all calls, interventions, care plan changes, physician contacts within the care coordination notes of EMR system in a manner consistent with policy and procedure
- Documenting accurate assessment of patient status and updating the plan of care utilizing appropriate communication (verbal, electronic, written, email documents and databases)
- Documenting patient care reflecting nursing interventions, patient response to care, focusing on the patient’s goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record
- Assisting patients, family members or caregivers with concern and empathy; respect confidentiality and privacy and communicate in a courteous and respectful manner
- Coordinating services with contracted vendors, outside agencies and interdisciplinary team as needed. Investigate and follow through on orders or requests for service or information
- Attending / Pronouncing patient deaths to provide support to the patient’s family, contact the mortuary, advise on disposal of narcotics, notify the IDT as well as contracted vendors providing services to the patient.
- Providing bereavement resources to the family as appropriate
- Responding to referrals calls, providing the caller with information regarding hospice care, screening calls for appropriateness, taking patient information and coordinating with the on-call supervisor to schedule the admission visit
- Performing patient admission visits as assigned by supervisor Reporting concerns to the hospice supervisor on-call
- Performing blood draws if required
- Providing and maintaining a safe environment for the patient
- Assisting the patient and family/caregiver and other team members in providing continuity of care
- Working in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.
- Attending interdisciplinary group meetings as needed
Qualifications + Skills of Hospice On-Call Nurse
- Valid Registered Nurse (RN) License to practice in Pennsylvania
- Current BLS/CPR certification
- Valid Pennsylvania Driver’s license
- Minimum of one year nursing experience is preferred
- Wound Care experience is preferred
- Med Surg, ICU, ER, Acute Experience, Home Health or Hospice experience is a plus

Pay: $70,000.00 - $78,000.00 per year
