Title: Companion
Purpose of Your Position:
Companions will accompany residents from our facility to their scheduled appointments to clinics, hospitals and other appointments required. They may need to meet the same residents on a regular basis and ensure their safe and comfortable transportation to and from the facility as scheduled.
We are looking for anyone with a flexible schedule, CNA certificate, and/or a CNA completion certificate. If you are newly graduates, you will have the opportunity to work with us after receiving your CNA certificate.
MUST BE AVAILABLE MONDAY TO FRIDAY
Major Duties and Responsibilities:
- Accompanying residents to and from medical appointments. Remain with the patient at all times.
- Assists residents, whether ambulatory or non-ambulatory.
- Observing and monitoring residents.
- Presenting medical conditions and concerns to the health care provider, and accurately reporting medical information from the healthcare provider back to the residential team.
- Assist with patient’s positioning and mobility.
- Ensures in patient care and patient safety while off facility.
- Maintains confidentiality of all Resident and Facility data.
- Ensures that patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage a complaint, are followed at all times.
- Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit.
- Maintains confidentiality of all pertinent patient care information to assure patients’ rights are protected.
- Reports and informs facility of any changes in resident’s situation or plan during visit.
- Other related duties as may become necessary or as directed by the Charge Nurse, Nursing Supervisor, Nursing Director or Administrator.
