Advanced Home Health of Arizona is looking for a full time Occupational Therapist to join our Home Health team in Scottsdale, AZ.
Our OTs plan and provide physical services to clients in their homes in accordance with the plan of care and Agency policies and procedures. The Occupational Therapist performs functions that require substantial specialized knowledge, judgment and skill and must be able to make judgments accordingly.
Qualifications
- Graduate of an accredited occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or is eligible for the National Registration Examination of the American Occupational Therapy Association.
- Possesses current registration or license as required by state(s) of practice.
- Minimum of two (2) year experience in home health or equivalent experience but we do welcome new grads.
- Demonstrated good written and verbal communication skills, interpersonal skills.
- Licensed driver with automobile insurance in accordance with state/Agency requirements.
- Good driving record and reliable transportation.
- Current CPR certification.
- Current finger print clearance card.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
- Performs occupational therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation of clients who are receiving services under a medically approved plan of care.
- Provides assessment findings and goals to assist in developing the client care plan and obtains specific physician orders as needed.
- Documents all findings, plans, interventions, and client progress (outcomes) on designed forms and submits per Agency guidelines, every forty-eight (48) hours.
- Communicates plans and changes to the physician and to nurse manager and other Agency staff through the care plan, written progress notes, and participation in case conferences.
- Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.
- Advises, consults, and teaches appropriate treatment procedures and supportive activities to clients, family members, and other Agency staff.
- Designs and/or orders appropriate adaptive/assistive devices
TYPICAL PHYSICAL DEMANDS
Requires a full range of body motion including the handling and lifting of patients. Manual and finger dexterity and eye-hand coordination required. Occasionally will lift and carry items weighing up to 50 pounds or greater. Requires corrected vision and hearing to normal range. May require working under stressful conditions or working irregular hours. Involves exposure to communicable diseases. Requires a current driver’s license and the ability to drive and access a patient’s home without an assistive device.
Pay: salaried position, offering between $80,000-$90,000 starting pay
