The qualifications:
- Current and experienced liaison working directly with or in Grand Strand hospital(s) and/or skilled nursing rehabs / clinical settings (prefer at least 2 years)
- Have been in the local Myrtle Beach / Grand Strand SC healthcare market for at least 2 years
- Good relationship builder, with passion for helping seniors and families
- Case Manager, Nurse Case Manager, Social Worker background a plus
The responsibilities:
- Obtaining referrals and handling case load with Case Managers
- Working with both new and established referral sources
- Work out of home, likely 75% in the market in the Grand Strand and 25% working at home
- Meet directly with seniors and families in clinical and home settings
