The purpose of the Director of Social Services position is to enable the facility to identify the personnel and medically related psycho- social, socio-economic and emotional needs of the residents. To act to meet those needs through individual and family assessments and counseling as well as other social services interventions. This position also promotes residents' potential, rights and quality of care during their stay and collaborates, when indicated, an appropriate discharge plan.
- Provides social work assessments for each resident, which identifies their personnel and medically related current situation and related history.
- Participates in interdisciplinary care plan conferences and contributes to the development of care plans for residents.
- Provides regular Progress and Quarterly notes in the residents' medical chart indicating the needs, changes , recommendations and concerns regarding the resident.
- Attends all meetings required by Medicare, Medicaid and the Facility to provide continuity of care and meet the in going needs of the residents and comply with regulatory guidelines.
- With the Administrator, interprets and promotes residents' rights and "Residents' Bill of Rights".
- Provides individual and group help and support for residents and families at times of adjustment, crisis or particular need.
- Orients residents and families to the facility environment, and assists in the adjustment of Both.
- Provides Quarterly or as needed up dates and verification regarding residents' DNR and all other Advanced Directives forms in order to insure that the current desires of the residents are accurately documented and will be followed in accordance to their wishes.
- Maintains an open door policy for all residents, their families and others who support their needs.
- May assist in the coordination of admissions as well as discharges with staff, residents, and families.
QUALIFICATIONS
- Must have a current Social Worker License from the State of Texas as a LSW, SWA, or LMSW.
- Skilled Nursing/AL experience REQUIRED
- One year of social work supervised experience in an individual care setting
- Similar related experience/professional qualifications
- Effective interpersonal and communication skills and ability to write clear and appropriate documentation
- The ability to visibly see the residents and physically be able to assist residents in cases of fire or other disasters'
