StrideCare is looking for a Nurse Care Coordinator that will be responsible for the programs and resources that support the identification, education, and navigation of patients towards best practices and outcomes. The Nurse Care Coordinator will collaborate with our Podiatry and Vascular teams to detect acute and chronic complications, navigate patients towards ideal interventions(s), and identify crucial care gaps in patients’ course of specialty healthcare. The Nurse Care Coordinator will be instrumental in helping develop our coordination program through:

Assisting with access to specialty referrals

Provide valuable input on processes and services

Tracking patient interventions and outcomes

Developing relationships with multiple providers and care teams to efficiently facilitate care for patients
The Nurse Care Coordinator will serve as a point of contact and act as a resource expert for patients, care givers, primary care, and specialty providers to ensure highest quality of care.

Essential Functions

Reviews patient lists, cases, and appointments generated by EMR and data triggers to identify patients that could benefit from intervention.

Reviews charts, results, and labs to identify potential care pathways and services to enhance provider value of care

Communicates and engages with physicians daily about patients’ potential for referral and additional treatment/services needed

Initiates and facilitates communication regarding EMR data triggers with the relevant care team members

Assists in facilitating, scoring, and communicating results of patient questionnaires
-
Coordinates information among referring, primary care, and consulting physicians and ancillary departments.

Educates patients and families regarding referrals, procedures, medications, and treatments.

Answers patient/family phone calls, fields questions appropriately.

Tracks outcomes of referrals and treatments.

Maintains detailed records to measure the effectiveness of the referrals and suggest ways to improve throughout the process.

Participates in creating an environment that supports the mission, vision, and values of the organization.

Encourages collegiality and collaboration when integrating pilot services with other departments.

Evaluates and provides leadership input regarding patient satisfaction

Evaluates and provides leadership input regarding effectiveness and utilization of resources to meet the needs of the target population.

Qualifications and Education Requirements

License as a Licensed Practical, Vocational Nurse or Registered Nurse in Texas

Experience documenting within an Electronic Medical Record is required

Clinical skills: care coordination, documentation of care, medication administration, assessing patient educational needs, obtaining vital signs, and patient screenings as needed.
