Qualified Intellectual Disability Professional (QIDP)

Minimum Requirements:
- Must be 18years or older
- Bachelor degree in a Human Services or related field,
- Must meet DHS requirements for QIDP in Illinois
- At least one year experience working with people with intellectual/developmental disabilities,
- Ability to perform basic physical moves such as kneeling, reaching, crouching, bending, and twisting,
- Possesses valid driver’s license and proof of insurance,
- Basic computer proficiencies,
- CPR and First Aid certification.
Job Responsibilities:
Administrative Responsibilities Includes but not limited to:
- Support the Mission Statement and agency values of Abequa Home Health Care,
- Demonstrate leadership abilities by working cooperatively with other Abequa staff,
- Promote teamwork, respecting differing viewpoints, and demonstrating the ability to solve problems creatively from an agency-wide perspective,
- Advocate for consumer rights and services,
- Ensure individual personal information is accurately and confidentially maintained,
- Represent the agency in a professional manner to the individuals, guardians, family members, community members, and other vendors and affiliated agencies,
- Initiate, coordinate and monitor to assure the Persons Center Plan for each individual assigned to caseload is completed in a timely manners,
- Ensure the participation of individual, guardian and designated family member,
- Assure services specified outcome as outlined the person plan are being provided and monitored,
- Provide necessary and adequate required IDHS training to PSW in a timely manners,
- Provide training to Personal Support Worker (PSW) in properly implementation and documentation of the outcome plans as outlined n the individual life choice areas,
- Maintain compliance with personnel policies as stated in the employee handbook,
- Demonstrate an understanding of the agency Compliance policy including the employee work rules and state/federal regulations related to services provided by the agency,
- Maintain monthly documentation on individual progress on persons plans as established in the outcomes,
- Maintain the 12 continuing education credits annually according to IDHS policy for QIDPs,
- Complete other duties as assigned by the immediate supervisor.
Consumer Interaction/Relationship:
- Approach individuals needs with kindness, courtesy, and consistency,
- Allow individual to make decisions and Respect person choices.
- Demonstrate knowledge of each Individual outcome and Persons Plan established for each period.
- Ensure that individuals are participating in the activities of their choosing at home and in the community as stated in the persons plan,
- Assist individuals with working on plans as established in the persons plans,
- Assist individual in learning the skills that will assist the person in achieving the greatest level of independence,
- Ensure that the individual medical and behavioral needs of the individual needs are met,
- Participate in the development of individual behavioral plan and be part of the human right committee if possible,
- Assist individual in obtaining support with conflict resolution and problem solving as needed on an on-going basis.
Job Type: Full-time
Pay: $24.00 - $27.00 per hour
Expected hours: 40 per week
