Community Health Worker

Description:
Familias Unidas program exists to improve wellness and self-sufficiency in Latinx and other communities. We accomplish this by delivering quality mental health counseling, service advocacy, and information/referral services. Our programs include: mental health, education and prevention, youth development, and wrap-around services. The Community Health Worker/Promotor will be responsible for providing mental health counseling and case management services to child, youth, and adult clients. They will also conduct assessments, develop treatment plans, and collaborate with other professionals and organizations to coordinate care
Minimum requirements:
- High School Diploma or equivalent req; 2-3+ years of related/equivalent experience in a similar position with training/certifications; 80hrs
- Valid California Driver’s License, Car insurance, and Car registration.
- Bilingual (Spanish/ English; speak and write).
- Pass FBI background check (Custodian of Records for DOJ)

- Attention to detail and accuracy in data entry and document management.
- Capable of working/ understanding the needs of a multicultural environment.
- Strong work ethics, maintaining a professional appearance, and exercising discretion with sensitive information.
- Ability to work in a fast-paced environment, handle interruptions, and adapt to changing priorities.
- Ability to perform tasks related to physical activity to include sitting, standing, for long periods of time, caring, bending, moving, and lifting up to 50 lbs
- Understand the community served and to be connected to the community.
Responsibilities:
- Provide mental health counseling and case management services to child, youth, and adult clients
- Conduct assessments, develop treatment plans, and collaborate with other professionals and organizations to coordinate care
- Conduct community-based health education sessions on topics such as preventive care, nutrition, chronic disease management, mental health, and substance abuse.
- Develop culturally tailored health promotion materials and resources for distribution within the community.
- Collaborate with healthcare providers to ensure community members receive accurate and up-to-date health information.
- Build and maintain relationships with community members, local organizations, and stakeholders to promote El Sol NEC's programs and services.
- Organize and facilitate community events, workshops, and health fairs to increase awareness and participation.
- Conduct door-to-door outreach to identify community needs and connect individuals with appropriate resources.
- Conduct follow-up visits to ensure continuity of care and address any barriers to health services.
- Assist with the coordination of support groups, peer counseling, and other community-based initiatives to improve health outcomes.
- Maintain accurate and confidential participant records, ensuring compliance with organizational and regulatory standards.
- Collect data on program outcomes, participant demographics, and community needs to inform program improvements.
- Coordinate services with health care and social services agencies.
- Complete and submit by the deadline all required agency and program documentation.
- Participates in staff meetings, individual supervision, and training related to their work.
- Must be able to use their own vehicle for travel.
- Ability to work occasional evenings and/or weekends as needed.
- Other duties as assigned.
