1. Overseeing the services (CLS, CAG) for each individual being served.
2. Attend ISP meeting and other meeting related to care of the individual.
3. Ensure services are being delivered through DBHDD and HFR requirements and including documentation.
4. Ensuring the development and updating of required policies for the agency.
5. Conducting functional assessments to support formulation of the individual’s plan for delivery of all waiver services.
6. Experience with auditing preparation and quality reviews.
7. Must be able to provide support, training, and supervision to direct care staff.
8. Read, correct, train staff on proper documentation.
9. Conduct supervisory visits, generate monthly and or quarterly reports.
10. Familiar with Therap (documentation system) is plus.
11. Must have a Bachelor Degree
12. A strong possibility to change status to full-time position after probation period.
Job Types: Part-time, Contract
Pay: $18.50 - $24.50 per hour
