will:
· Uses learned pu
ic health nursing practice, theories, and principles to provide direct services to individuals, families, special groups, and schools in the center, home, and community environment in the Community Health Center.
· Utilizes knowledge in appropriate referral systems and acts as a facilitator to complete referral process.
· Completes the required documentation of patient care for records and documents as required by the agency’s billing process and according to grant guidelines.
· Maintains statistical data as required.
· Provides professional nursing care according to physician’s orders in conformance with recognized nursing techniques and procedures, established standards and administrative policies of the Health Department and the Community Health Center.
· Must complete blood draws and Orientation Checklist supervised by Clinical Nursing Coordinator and CMO.
· Work with the public; exhibiting courteous behavior, non-judgmental attitude, being culturally aware and maintaining client confidentiality.
· Operate office machines, computer equipment, and telephone system as needed.
· Provide assistance in front desk functions, as appropriate, scanning and scheduling appointments.
· Monitor inventory of supplies for Community Health Center, assist in re-ordering, when necessary, and ensure exam rooms are appropriately always stocked with medical supplies.
· Work with the public, exhibiting courteous behavior, non-judgmental attitude and
maintaining client confidentiality.
· Performs all other duties as assigned.

*Requirements:*
· Certification of graduation from an accredited school of nursing; and a minimum of Illinois license as a Licensed Practical Nurse.
