Job Summary
Our ideal candidate has a strong working knowledge of cardiology, heart failure, and cardiac implantable electronic device (CIED) management principles. A unique remote work opportunity, this role provides the chance to serve patients by proactively monitoring, coaching, and reporting relevant arrhythmia and heart failure alerts to the following provider. Early intervention through CIED remote monitoring reduces risk for emergent care and/or hospital admission/re-admissions. As a leader of the care team interacting with patients and allied health professionals, this position will provide valuable feedback on best practices while participating in the cardiology care continuum.
Duties/Responsibilities
- Responds to cardiac device alerts pertaining to arrhythmia events or changes in heart failure physiologic data
- Contacts patients to obtain subjective data regarding patient symptoms, medication adherence, and other relevant patient information.
- Reviews objective data in patient’s medical record: medication list, diagnoses, laboratory results, diagnostic tests, upcoming appts
- Adds a nursing note to the device transmission report that documents subjective and objective data
- Communicates alerts and findings to the cardiology provider and clinical team
- Able to respond to provider orders and changes in plan of care; communicate changes to patient, document in the electronic medical record
- Manage patient messages and respond
- Interact and work closely with CIED clinical team members
- Develop standard operating procedures (SOPs) and best practices for care delivery
- Guide team with clinical expertise in patient data interpretation for cardiology and Heart Failure patients
- May be required to perform other duties as assigned
- Weekend and/or holiday on-call rotation as needed
Required Experience, Qualifications, and Skills
- Unrestricted RN license in South Dakota
- Preferred RN license in Compact States, or the ability to obtain licensure in these states
- Strong cardiology clinical background with 5+ years nursing experience.
- Cardiac device experience preferred
- Strong interpersonal and communication skills. Comfortable communicating with patients and caregivers, physicians, and other clinicians.
- Strong personal and professional judgement
- Capable of working independently and demonstrates self-motivation
- Excellent documentation skills
