We are looking for a compassionate and driven registered nurse to provide the highest-quality care for patients in their homes.
The Clinical Supervisor is responsible for reviewing patient assessments to ensure appropriate and effective care is provided to each patient in compliance with agency policies, procedures, and regulatory requirements. Trends or patterns of inaccuracy identified by the Clinical Supervisor will be communicated to the Home Health Administrator and VP of Operations to assist in the development of the performance improvement and education plan.
Responsibilities + Duties
- Oversee and help manage the multi-disciplinary personnel to ensure that the highest quality of care is being provided consistently to all patients
- Review electronically submitted documentation and provide feedback to clinicians regarding deficiencies or OASIS corrections to ensure the medical record reflects compliance with skilled need, medical necessity, and homebound status if required
- Ensure visit utilization is supported by individual patient assessment
- Review additional orders for appropriateness
- Ensure clinical documentation supports Recertification/Discharge decision
- Review and approve final Plan of Care (485) within Agency’s EMR
- Review OASIS for inconsistencies and lock timely
- Perform Patient Visits on an as-needed basis
- Fully knowledgeable of all applicable federal, state, local and Medicare regulations
- Exhibits Management skills, advises, guides and inspires the nursing team to perform their best and continually improve communication and job performance for the health and well-being of patients
- Perform yearly evaluations and competencies
- Oversee new admissions, carrying out new physician orders and development of comprehensive patient care plans
- Ensure that the agency is consistently survey ready and regulatory compliant
- Strong ability to problem-solve with patient/family concerns, issues and conflicts
- Assess multi-disciplinary staff’s needs and sets obtainable goals to enable the agency to provide excellent clinical services
Qualifications + Skills
- Valid RN license in the state of Pennsylvania
- Minimum of 1 year of home health experience with a working knowledge of Medicare Home Health Conditions of Participation and OASIS
- Knowledge of all applicable federal, state, and local regulations
- 2 - 3 Years of RN Nursing Experience
- Experience Managing a Team
- Very Strong Nursing experience
- Strong Organizational Skills
- Excellent Verbal and Written Communication
