Position Overview: At the highest level of, OpusCare of Texas Employees must a) be committed to hospice philosophy and mission and b) Provide outstanding customer service to both internal and external customers. The C.N.A. works to increase the level of comfort, and maintains personal hygiene and a safe, healthy environment for the patient. As a member of the interdisciplinary care team the C.N.A. is responsible for providing personal care services and for performing related tasks, for the terminally ill patient, in the patient/family place of residence, in accordance with a written assignment (plan of treatment) by the Primary Field Nurse Supervisor. Must be available to travel our service area.
Essential Duties:
Adheres to established standard infection control procedures in the home environment.
Assists each specific patient with the prescribed activities documented in the nursing assistant’s plan of care, for that patient.
Provides or assists patients with personal hygiene and grooming, including bed bath, sponge, tub or shower bath, shampoo (sink, tub or bed) nail and skin care, u se of skin emollients, oral hygiene and dressing activities.
Assists patients, in a safe manner, with transfers, in-out of bed, ambulation, and to use the bathroom.
Provides and assists patients and families with proper positioning of patient in bed and/or char for comfort measures.
Prepares and/or assists with the preparation of a light meal, breakfast or snack, in the patient’s home, and assists with the patient’s feeding, when necessary, in his home environment.
Supervises patients self-administer their own medications, as needed.
Assists patients with the use of the bedpan or urinal.
Performs household services which will facilitate the patient’s self-car at home and are necessary to prevent or postpone institutionalization.
Changes the patient’s bed linen and maintains the patient’s room in an orderly and clean manner.
Provides continuous care, pursuant to a schedule, to individual patients, as requested during periods of crisis.
Provides observation of patient’s condition, on every visit, reporting changes in the patient’s condition to his/her nurse supervisor.
Completes appropriate records and ensures such records are turned in to the office timely.
As a member of the Hospice program interdisciplinary care team, provides feedback to other team members of his/her observations and finings.
Maintains a nurturing attitude towards patients and families being serviced, to include ensuring personal respect towards the patient and family members, and their privacy and property.
Respects patients and families’ wishes regarding their grieving process, providing supportive attitude at all times.
