Job purpose
To provide clinical services to adults and families and to be available to assist with the agency’s clinical needs.
Duties and responsibilities
1. Maintains client relationships, which ensures that each individual is treated with respect and has the opportunity to learn and grow in a positive and supportive environment.
2. Completes weekly clinical progress notes to treatment process; description of services rendered and what goals were addressed.
3. Completes a thorough discharge within 72 hours noting appropriate elements.
4. Utilizes evidenced based treatment models and utilizes clinical outcome measures, gathering and analyzing the data per state requirements.
5. Utilizes clinical outcome measures on admission and every 3 months thereafter.
6. Identifies quality indicators for each client and provides appropriate data when needed.
7. Reports emergencies in a timely manner to appropriate parties.
8. Ensures that communication among all relevant parties takes place in an accurate and timely manner.
9. Attends weekly individual and group supervision as needed.
10. Establishes a clear understanding with guardians regarding frequency and content of communication and positive working relationships.
11. Establishes and maintains positive relationships with clients, case managers, legal guardians, and family members.
12. Demonstrates knowledge of defined best practices for co-occurring disorder treatment; employs diverse theories, models and intervention methods.
13. Ensures that all clinical documentation is complete and accurate and that client records are properly maintained and properly secured.
14. Role models clinically appropriate client interactions for Direct Support Professionals (DSP).
15. Completes comprehensive bio-psychosocial assessment/Services assessments within 30 days of admission and annually as needed.
16. Completes written plans and oversight and training to Direct Support Professionals (DSP)
17. Provides on-site mental health treatment.
18. Participates in quality assurance audits under the direction of the State Director.
19. Performs other related duties as requested.
Qualifications
Candidates must have an LCSW or BCBA. Candidates will need to possess a strong clinical understanding of intellectual and developmental disabilities along with a deep understanding of childhood mental health issues and trauma. Ability to drive for the agency.
Working conditions
This is a hybrid position. There may be travel required to the individual’s residence. We are seeking a PT clinician 10-20 hrs. a week.
