Job Overview:
We are seeking a dedicated Home Health Physical Therapist to join our healthcare team. The ideal candidate will provide quality care and support to patients in need of occupational therapy services.
Duties:
- Conduct patient assessments to determine individual needs and develop customized treatment plans
- Implement occupational therapy interventions to enhance patients' independence and quality of life
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Monitor and document patient progress and adjust treatment plans as needed
- Provide education to patients, families, and caregivers on therapeutic techniques and strategies
Qualifications:
- Bachelor's or Master's degree in Physical Therapy from an accredited program
- Valid state licensure as an PhysicalTherapist
- Strong knowledge of anatomy, medical terminology, and medical imaging
- Experience working with diverse patient populations with a focus on geriatrics
- Proficiency in patient care, case management, and occupational health practices
