Crystal Home Care is hiring and immediately in need of filling the open position of a Care Coordinator for our facilities/buildings located in the Burr Ridge, Brookfield IL area.

Responsibilities:
· Contacts client within 24 hours of receiving the lead and schedules an appointment for home visits.
· Conducts home visits to assess Community Service needs. Identifies client needs and develops a service plan to meet those needs, such as personal care, medication management, and companionship, etc.
· Communicates with all involved parties such as family, partners within community, and Crystal Home Care office staff.
· Establishes, manages, and oversees a detailed, multitasked plan to deliver the services to the client and coordinating as a liaison between the facility and partners in care such as hospitals and clinics, to facilitate a smooth transition for new clients.
· Organize and host onsite events and activities that can attract potential clients, like health workshops, open houses, or community gatherings.
· Use these events as an opportunity to engage with attendees and discuss the services offered by CHC Communicates with the Director and Office Manager details of the service plan, staff needs, partner needs, special requests, etc. Monitors the service plan, adjusting the plan as needed.
· Confirms with all parties’ expectations and timelines. Provides support, reassurance, and problem-solving to achieve client satisfaction- Is respectful of clients’ possessions and home environment.
· Assists in the preparation of daily, weekly, and monthly reports as needed. Provides data input to various software programs as the department requires.
· Responsible for meeting the licensure requirements for documentation. Follows and promotes confidentiality involving client and family interests Develop strong relationships with residents and their families, understanding their needs and preferences.
Physical Requirements and Work Environment:
· The employee frequently is required to use manual and finger dexterity and eye-hand coordination when handling office equipment (e.g., computers, copiers, etc.).
· Requires corrected vision and hearing within normal range.
· The noise level is normally quiet.
· Typical residential/business environment.
· The work schedule may include working days, evenings, weekends, and holidays.
Qualifications:
Overall
· Bachelor’s degree required with 1-2 years’ experience as a coordinator in a healthcare setting required or an equivalent combination of education and experience from which comparable knowledge and abilities can be acquired.
· Prior “Wellsky” or other patient care software experience preferred.
· Proficient in Microsoft Office products and customer relationship management tools.
· Excellent interpersonal communication and customer service skills.
· Ability to maintain confidentiality and handle sensitive information with professionalism.
· Excellent attention to detail and accuracy in data entry and record keeping.
· Strong organizational skills with ability to prioritize tasks effectively.
· Ability to work in a group and independently with little supervision.
· Self-starter and quick learner.
· Strong desire to work with older adults and be able to establish a relationship of trust and confidence.
Job Type: Full-time
Pay: From $50,000.00 per year
