We need coverage for patients who live in the Robertson County/Goodlettsville TN area.
In collaboration with a supervising physician, the Nurse Practitioner/ Physician Assistant expands the individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions for our transitional care/palliative care organization. Transitional care is a combination of disease modification, primary care and management of distressful symptoms in individuals with severe illness—recently discharged from either an acute care facility (hospital), post-acute care facility (SNF) or long-term care facility.
POSITION RESPONSIBILITIES
- Make a visit to the patient's home within 24/48 hours of discharge
- Make a 7 day follow up visit to the patient's home
- Make a 14 day follow up visit to the patient's home
- Medication reconciliation
- Transition to adjuvant pain treatment
- Manage symptoms, including scheduled Rx for the first week, nausea, dyspnea, mood
- Bowel/bladder management (cathartics, depends, etc)
- Maintains communication with the primary care provider
- Provides continuity across time and space of multiple care levels
- Coordinates and maintains continuity with secondary services (home health, rehabilitation facilities)
- Confirm HHA efficacy and availability of DME (respiratory care, IV therapy)
- Refers to secondary providers for difficult and/or complex management issues (psychiatric illness)
- After 30 days, the patient will resume care with their PCP, if they are Transitional. If they are Palliative, you will see them once a month, as needed.
We currently need Transitional Care, Post Emergency Room coverage to see our patients leaving the inpatient setting within 7 days, Palliative Care, Acute Home Visit care in the Davidson Co. Area
POSITION QUALIFICATIONS & LICENSE Required Licenses/Certifications/Registration
- Nurse Practitioner specializing in Advanced Practice obtained prior to hire date or job transfer date
- DEA
- One or more of the following:
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing
- Current home state license for multi-state license recognition "compact state" obtained prior to hire date or job transfer date.
- BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date
POSITION EDUCATION
- Master's Level Degree or MSN from an accredited program approved by the State Board of Nursing program
- Certified as a Nurse Practitioner in the state of TN
POSITION EXPERIENCE
- Minimum of 3 - 5 years of clinical nursing experience- CCU/ICU preferred.
- 1-2 years of Nurse Practitioner experience preferred.'
