Job Summary:
Responsible for supportive services and building safety for 20 units of permanent supportive housing located at 1300 Vine Street (buddy’s place). Assist participants with housing placement, housing retention, accessing community services and resources, and developing independent living skills for residents of the project. Monitor building to ensure safety of all residents and guests through the implementation of a Housing First and Harm Reduction model.
Qualifications and Requirements
- Licensed Social Worker (LSW), bachelor’s degree and/or experience working with populations experiencing homelessness and histories of substance abuse and/or mental illness
- Supports Mission of OTR Community Housing
- Valid driver’s license and insurance, use of personal vehicle
- Proficient knowledge of Microsoft Word and Outlook and experience with computer database programs
- Excellent written and verbal communication skills
Essential Functions
- Provides supportive services to 20 residents at buddy’s place
- Assists with resident intake and assessments
- Works with CoC Coordinated Entry System and CMHA Intake process
- Addresses client barriers through motivational interviewing skills
- Actively works with client in developing individual service plan and goals
- Works on an ongoing basis to assist client in achieving specific goals, including maintaining housing and increasing income
- Makes appropriate referrals to access community resources
- Maintains system for recording resident data and services provided, including accurate data input in the Homeless Management Information System (HMIS)
- Documents and evaluates client progress in timely, accurate manner
- Monitors that all building policies, procedures, and rules are clear and applied in a consistent manner that contributes to a calm, peaceful and nurturing environment
- Submits building work orders to the main office in a timely manner
- Coordinates unit turnovers with maintenance staff
- Provides transportation or transportation assistance for residents as appropriate and as funds are made available
- Holds periodic house meetings with all residents
- Conducts periodic inspections or wellness checks of units
- Performs all other duties as needed or assigned by supervisor
Communication
- Works closely with OTRCH Property Management staff
- Actively works to stay informed of community resources that may assist residents
- Attends regular meetings with Supervisor
- Able to use motivational interviewing skills to engage residents
- Maintains professional working environment and professional relationship with residents
- Regular and appropriate use of technology, including e-mail, internet, and cell phones
- Document all appropriate activities and incidents
