POSITION*
Skill Care Nurse Manager
POSITION PURPOSE
Supervises licensed practical nurses (LPNs) and private duty nurses (PDNs) to ensure that the care delivered is high quality, meets the needs of the client, meets or exceeds regulatory standards, is financially sustainable, and results in satisfied consumers. Collaborates with other departments to streamline the onboarding of clients and their care matched nurse. Manages nurses’ orientation, training, schedules and performance. Performs skilled nursing services, provides consumer nurse education, completes required training and competency documentation of supervised staff.

KEY DUTIES & RESPONSIBILITIES*
- Manage Community Care Nurses
- Oversee nurses' performance and schedules.
- Orientate and validate new nurses' skills.
- Assign nurses to clients, adjust caseloads, ensure coverage, and review timesheets.
- Review referrals and notify staff of hospitalizations and discharges.
- Handle client calls and complaints, provide feedback, and conduct performance evaluations.
- Communicate updates and regulatory information.
- Support and motivate the nursing team.
- Ensure High-Quality Care
- Monitor clinical skills and knowledge, oversee competency testing
- Analyze care quality, identify areas for improvement.
- Ensure adherence to orders and care plans, analyze outcomes, and coordinate care.
- Act as a clinical expert, lead quality improvement initiatives.
- Ensure Compliance with Regulations:
- Hold nurses accountable for paperwork, ensure compliance with regulations.
- Monitor performance, investigate incidents, and participate in quality improvement initiatives.
Review clinical documentation, follow up on tasks, and handle challenging client conversations.

- Manage Mentor RN:
- Develop role, delegate projects, and provide support.
- Assist with content planning and delivery.
- Maintain Protocols:
- Update clinical protocols and administrative policies.
- Interact and Collaborate:
- Collaborate with peers and managers to refine processes and solve issues.
- Support outreach efforts, represent PASCO as a clinical expert.
- Other Responsibilities:
- Lead meetings, perform assigned duties.
- Perform skilled nursing services under the orders of the consumer’s physician and in accordance with accepted standards of practice and governing body rules and regulations
- Adhere to responsibilities defined in the Conditions of Participation.
- Provide oversight of client care services, coordinate referrals, and ensure documentation compliance.
LOCATION/SCHEDULE: This position is hybrid working both in-office (Lakewood, CO) and remote.
PAY: 75K - 85K DOE
