Basic Function:
The MDS/Care Plan Coordinator is responsible for coordinating the completion of the MDS and the Interdisciplinary Plan of Care for all residents admitted to the facility. The individual also establishes and maintains positive relationships with other members of the health care team.
Essential Functions:
- Develops individualized plan of care for all residents. Maintains current MDS status of every resident, according to state and federal regulation.
- Schedules residents for appropriate admission, annual, quarterly, or change of condition assessments, overseeing communication to nursing and other departments.
- Responds to resident change in condition by coordinating resident reassessments and reevaluating plan of care.
- Assures compliance with Federal and State regulations requiring completion/coordination of the comprehensive assessment (MDS).
- Monitors MDS and care plan documentation on all residents, assuring that complete, orderly, and chronological documentation is in file.
- Maintains proficiency in software program.
- Communicates information to nursing administration and facility administration regarding care plans, updates and revisions.
- Coordinates and attends interdisciplinary team™ meetings, attends Medicare, weight assessment and other meetings in order to gather information, communicate changes, and maintain updated records.
- Remains current with changes in the industry through attendance at outside workshops and conferences.
Minimum Skills Required:
- Excellent patient assessment skills customarily obtained through prior experience to nursing services.
- Ability to communicate effectively in verbal and written form.
- Ability to work independently as well as a meaningful member of a team.
- Excellent organizational skills in order to coordinate many projects simultaneously. Ability and willingness to set goals, monitor progress, and obtain established goals within established time frames.
- General clerical skills, including typing, telephone, and basic computer skills.
- Excellent attention to detail.
Minimum Requirements:
- B.S.N. preferred
- LPN required
- Current RN and/or LPN license
- Must have knowledge of RAJ process and proven MDS experience.
Work Experience:
- 5+ years direct hands-on nursing experience
- 2+ years long-term care experience
- Ability to utilize both local and corporate resources in the execution of job responsibilities.
- Ability to develop an adequate knowledge of State, Federal and other regulatory requirements related in the network.
