Job Overview:
The Social Worker position offers an exciting opportunity to have a positive impact on the Sunset Park, Brooklyn community through engaging with clients through RaisingHealth program development and direct service delivery. This role is pivotal in delivering exceptional clinical support to our clients, helping them navigate through various challenges and ensuring their overall well-being.
As part of the interdisciplinary team, the Social Worker is responsible for conducting in-depth health risk assessments covering social, physical, and environmental concerns for adults and families. S/he will identify the client’s needs and provide follow-up to connect them to available community resources. Engagements will focus on addressing barriers to consistent care, resources, and treatment.
Responsibilities include, but are not limited to:
- Manages a regularly assigned caseload of community members using various approaches, motivational interviewing, and goal setting, while evaluating progress, and developing/modifying treatment plans accordingly.
- Provides timely psychosocial assessment, social case work and linkage to community resources for clients who have chronic, life threatening or altering diseases and disorders. Advocates for services and resources for the underserved immigrant population in Sunset Park, Brooklyn to help them maintain an optimum level of health.
- Conducts timely and appropriate assessment and needs identification, prioritizing clients based on their needs. Assesses clients for psychosocial, financial, family issues, home safety, etc. that may be barriers to care.
- Facilitates discussions with clients to support and advise on health and/or social needs and resources available.
- Engages in short-term interventions through the coordination of referrals and service including but not limited to resources for food and/or housing assistance, shelter placement, medical care insurance enrollment, etc.; Utilizing our network of community partners to advocate for clients and refer to specialized services
Minimum Requirements:
- Current NYS LCSW or LMSW license required.
- 1-2 years of experience as a Social Worker in a community-based organization.
- Computer literacy: Solid command of MS Office; Experience with Electronic Medical Records (EMR) or client record databases a huge plus.
- Bilingual (Spanish/Mandarin (Chinese) a_ highly preferred. _
- Ability to work Tuesday through Saturday from 10am – 6pm.
